INITIAL INTAKE SESSION

The initial session will consist of completing some background information and assessments, reviewing Informed Consent, Professional Disclosure, Release of Information, Confidentiality and Limits, Expectations and Purpose of Counseling, Risks of Psychotherapy, and Session length (50 minute hour). Fee for Initial Intake Session is $100.00.

FEES

My fee for a counseling session is $100.00 an hour(50 minute hour). Payment is due at the time of service. It is important to me that financial considerations do not become an obstacle to your accessing services. Feel free to discuss financial issues with me.

No credit cards accepted. Checks and cash accepted.

My fees for evaluations, assessments, consultations will be provided to clients as necessary.

In the event that you need to cancel an appointment, please do so 24-hours in advance of your scheduled appointment. If the cancellation is not done 24 hours in advance there will be a charge of $50.00. If I am a network provider for your insurance, the charge will be your co-pay.

INSURANCE REIMBURSEMENT

Insurance companies have different requirements and guidelines for covering mental health and/or evaluation fees.

Please check with your insurance company to find out what they will pay for mental health service prior to your visit and to see if you need pre-authorization

If I am a participating provider for your insurance company, your payment will be your co-pay, deductible, or percentage. Please be sure to get a pre-authorization letter before your visit. Please bring your insurance card with you.

If I am not a participating provider for your insurance company, I will be happy to provide a statement for you to send to your company for any reimbursement or deductible fee that they might provide for you.
